Persons providing information to Perkins recognize that non-public
information concerning such person disclosed by such person to Perkins
or its agents (the “Information”) (such as such person’s name, address,
social security number, assets and income) is maintained in accordance
with Perkins’ Privacy Policy. Please download this PDF for

to Perkins' Privacy Policy, Information may be disclosed (i) to agents
of Perkins, including without limitation, attorneys, accountants,
administrators and auditors in furtherance of Perkins’ business and to
other service providers such as brokers who may have a need for the
Information in connection with providing services to Perkins and its
clients, including, without limitation, private investment entities
that have retained the services of Perkins (collectively, the
“Clients”), (ii) to third party service providers or financial
institutions who may be providing marketing services to the Clients
provided that such persons must agree to protect the confidentiality of

providing services to the Clients and (iii) as otherwise required or
